The apprentice idea is what I was thinking it was. Maybe that's how new reapers happen - dead souls who don't move on, don't turn vengeful, instead start learning.

Admittedly, I expect Death has final say, but with the growing world population, the occasional new reaper to the roster is probably something he has no problem with, since it keeps the job going smoothly.
